Insurance

I am an out-of-network physician for all insurance companies.

Payment

Payment is due at the time of the appointment (cash or check). A statement is provided upon request. You may use this statement to file a claim with your insurance company.

Cancellation Policy

I have a 24 hour cancellation policy. You will be responsible for the full fee for any appointment that is cancelled less than 24 hours in advance of the scheduled time.
